In a perio chart the clinician measures the pocket depth at six sites around each tooth with a probe and records the reading in millimeters. Readings of 1 to 3 mm are generally considered healthy; deeper pockets, bleeding on probing and recession indicate disease. Clinical attachment loss is calculated from pocket depth and recession.
Because a full chart means up to 192 readings, entry speed matters. Software offers keyboard entry, voice-activated charting and, more recently, AI that fills the chart from spoken numbers.
